Who We Are
A program of Horizon Behavioral Health, the Child and Family Center serves children, adolescents, and some young adults through the age of 21 with case management services. Services are offered in the home, schools and at multiple Horizon Behavioral Health location.
Services include:
- Intensive outreach;
- Education;
- Monitoring;
- Advocacy;
- Linking and needs assessment with goal of enhancing coordination of service delivery.

Intake Process
Call for a required appointment. Same day assessments for all ages are available Monday through Friday from 8:30AM-3:00PM. Same day assessments at the Bedford center are available on Mondays only.
