Who We Are

Harrisonburg-Rockingham Community Services Board provides case management assistance to individuals and their families with a focus on health and functionality for the people we work with and their support systems. Staff assess the situation and needs, link to appropriate services, and monitor care and conditions. The goal is to help people maintain their independence while living with severe and persistent behavioral health disorders. Case Managers work with supporting agencies and community resources to get the right service to the right person at the right time.

Case management professionals provide coordinated care to support at-risk people by assessing, navigating, and linking to the right resources to allow individuals to live in their communities. Case managers collaborate with other staff including psychiatrists, nurses, therapists, other qualified professionals and community partners. Case managers partner with other agencies such as schools, primary care physicians, specialists, the Department of Social Services, the Social Security Administration, etc. to help individuals receive the appropriate services to meet their specific needs.

Intake Process
Walk-ins are available at the Community Services Board main location. No appointment is necessary, however if interpretive services are needed please contact us to schedule an appointment.
