Case Management is a service that supports your treatment by helping you figure out what services you need, empowering you through person-centered planning. It links you to needed resources and community services and provides community-based support to help you get the most out of your recovery efforts.
Case management can help you gain control of everyday problems, help you express your needs, make life less frantic, confused and overwhelming, and help you manage your recovery.
Services in Lee, Scott, and Wise Counties and the City of Norton are provided in partnership with Planning District One Behavioral Health Services.
